ÖZ:
Çalışmamızın amacı otizmli çocuklarda duyu bütünleme eğitiminin
propriosepsiyon duyusu, motor performans ve duyu profili üzerine etkisini
incelemekti. Çalışmaya KKTC‟de yaşayan, araştırmaya katılmaya gönüllü olan 5-10
yaşları arası otizmli çocuklar (n=18) ve ebeveynleri alındı. Çalışma Ela Özel Eğitim
Merkezinin Fizyoterapi ve Rehabilitasyon Bölümünde ve Mağusa Fizyoterapi ve
Rehabilitasyon Merkezinde yapıldı.
Bireylerin Sosyo-demografik ve klinik bilgileri kaydedildi. Otizmli
çocukların duyu işleme becerilerini değerlendirmek için Duyu Profili, eklem
pozisyon duyusunu değerlendirmek için Propriosepsiyon Testi, motor becerilerini
değerlendirmek için Bruininks-Oseretsky Motor Yeterlilik Testi‟nin İkinci
Sürümünün Kısa Formu (BOMYT-2 Kısa Form) kullanıldı. Otizmli çocuğun bakım
vereni ve terapistin ortak kararıyla tedavi hedefi belirlendi ve Hedefe Ulaşma Ölçeği
kullanılarak kaydedildi. Değerlendirmeler tamamlandıktan sonra çalışma kriterlerine
uygun olan bireyler tedavi programına alındı.
Çalışmamız 18 otizmli çocuktan 15‟i ile tamamlandı. Propriosepsiyon duyu
testi sonuçları incelendiğinde eğitimi öncesi ve sonrası propriosepsiyon duyu kaybı
durumları arasında istatistiksel olarak anlamlı bir fark olmadığı saptandı (p>0,05).
Çocukların motor becerilerini değerlendirmek için kullandığımız BOMYT-2 eğitim
öncesi ve sonrası sonuçları incelendiğinde, bu testin alt basamaklarından olan ince
motor beceri hassaslığı, ince motor beceri bütünleşmesi, denge, ince motor kontrol
ve vücut koordinasyonu sonuçlarında istatistiksel olarak anlamlı fark bulundu
(p<0,05). Duyu profili sonuçları incelendiğinde ise duyusal işleme ana alt
parametrelerinden dokunma işlemi, hareket ve vücut pozisyonu ile ilgili
düzenlemeler, aktivite seviyesini etkileyen hareket düzenlemeleri, duygusal-sosyal
cevaplar sonuçlarında istatistiksel olarak anlamlı bir fark olduğu bulundu (p<0,05).
Davranışlar ve duygusal cevaplar alt parametrelerinden araştırma, hassasiyet,
kaçınma parametrelerinde ve modülasyon alt parametrelerinden ise duygusal girdi
arama, duygusal tepki ve zayıf kayıt bölümlerinde anlamlı derecede farklılıkların
olduğu bulundu (p<0,05). Hedefe ulaşma ölçeğinde belirlenen hedeflerin %26,7‟
sinin ilerleme gösterdiği diğerlerinin tedavi öncesi seviyelerini koruduğu belirlendi.
Otizmli çocuklarda uyguladığımız proprioseptif duyu bütünleme eğitiminin
propriosepsiyon duyu testi sonuçlarını etkilemediğini fakat duyu profili ve BOMYT2 testinin alt parametrelerini etkilediği bulundu. Tedavi hedeflerinde ise ilerleme
olduğu belirlendi. Tüm bu sonuçlar göz önüne alındığında duyu bütünleme
eğitiminin otizmli çocukların rehabilitasyonunda çok yönlü etkilere neden
olabileceği düşünülmektedir.

The purpose of our study was to examine the effect of proprioception, motor
performance and sensory profile after sensory integration training in autistic children.
Autistic children (n = 18) aged between 5-10 and their parents living in the TRNC
who volunteered to participate in the research were included in the study. The study
was conducted at the Department of Physiotherapy and Rehabilitation of the Ela
Private Education Center and at the Famagusta Rehabilitation Center.
Individual sociodemographic and clinical information at the participants was
recorded. Sensory Profile and Proprioceptive Test was used to assess sensory
processing skills and sense of joint position, respectively. Bruininks-Oseretsky Test
of Motor Proficiency, Short Form of Second Edition (BOT-2, Short Form) was used
to determine motor skills. The aim of the treatment was determined by the autistic
child's core giver and the therapist and recorded by using the Goal Attainment Scale.
Once the assessments were completed, individuals who met the study criteria were
included in the treatment program.
18 children who were diagnosed with autism were participated in our study.
However, only 15 children were completed the study. When the proprioceptive
sensory test results were examined, it was found that there was no statistically
significant difference between proprioceptive sensory loss before and after the
training (p>0,05). When the results of the BOMYT-2 treatment we used to assess
motor skills of the children were examined, a statistically significant difference was
found in the results of fine motor skill sensitivity, fine motor skill integration,
balance, fine motor control and body coordination subparameters (p<0.05). When the
sensory profile results are examined, statistical significance in the results of tactile,
movements and body position, behavioural patterns affecting activity level,
emotional-social responses were found (p <0,05). Behavioral and emotional
responses revealed significant differences in the search, emotion, avoidance
parameters, and modulation sub-parameters of emotional input search, emotional
response and weak recording sub-parameters (p <0.05). On the Goal Attainment
Scale, 26,7% of the determined targets had improved and others were determined to
maintain pre-treatment levels.
In our study, sensory integration training in autistic children did not affect the
proprioception sensory test results but found to affect sub-parameters of the Sensory
Profile and BOMYT-2. It has been determined that the treatment goals improved.
When all these results are taken into consideration, it has been found that the sensory
integration training has a multifaceted effect on the rehabilitation of children with
autism.
