The aim of this study is to compare the reaction time, static and dynamic balance, fall and functional mobility of subjects who were normotensive and diagnosed with hypertension, and to investigate the relationship between balance and fall risk of the hypertensive (HT) subjects.
The study included 64 normotensive and 64 hypertensive subjects who currently reside in Turkish Republic of Northern Cyprus. To incorporate all subjects in the study according to the required criteria, after measuring their blood preasure, the Standardized Mini Mental State Examination and the International Physical Activity Questionnaire for cognitive function and physical activity measurements, respectively were employed. The choice Stepping Reaction Time Test for reaction time, standing on one leg test for static balance, the Y Balance Test for dynamic balance, the Fall Efficacy Scale for fear of falling and the Timed Up and Go Test for functionaly mobility evaluation were used.
According to the results, reaction times of the HT subjects were statistically longer than reaction times of the normotensive subjects. The static balance (eyes open and closed) ( p’s <= 0.001) and the dynamic balance of the HT subjects were statistically lower than the normotensive subjects. Also, the HT subjects required longer periods for single and dual (cognitive and motor) tasks of the TUG Test, and more falling narratives compared to the normotensive subjects. Futhermore, the relationship between the balance and falling narratives of the hypertensive subjects were investigated and observed that there is a positively insignificant relationship (all p’s<0.05).
This study demonstrated that the HT subjects had longer reaction times, lower static and dynamic balance, more falling narratives and effected functional mobility with respect to the normotensive subjects. Thus, the authors concluded that the hypertensive individuals must be incorporated in either rehabilitaion programs or home-based exercise programs or exercise programs under a physiotherapist’s supervision.
ÖZ: Bu çalışmanın amacı hipertansiyon tanısı almış ve normotansif bireylerin reaksiyon zamanı, statik ve dinamik denge, düşme ve fonksiyonel mobilite açısından karşılaştırılması ve hipertansiyon tanısı almış bireylerde denge ve düşme öyküsü arasındaki ilişkiyi incelemektir.
Çalışmaya Kuzey Kıbrıs Türk Cumhuriyeti’nde yaşayan Hipertansiyon (HT) tanısı konmuş 64 kişi ve normotansif olan 64 kişi olmak üzere toplam 128 kişi dahil edilmiştir. Bu çalışmaya tüm bireylerin dâhil edilebilmeleri için gerekli kriterler kan basıncı ölçümü yapıldıktan sonra, bilişsel düzey değerlendirmesi için Standardize Mini Mental Test (SMMT) ve fiziksel aktivite ölçümü için Uluslararası Fiziksel Aktivite Anketi (UFAA) kullanıldı. Reaksiyon zamanının değerlendirmesi için Seçenekli Adım Alma Reaksiyon Zamanı Testi, statik denge için tek bacak üzerinde durma testi, dinamik denge için Y denge testi, düşme korkusu için Tinetti Düşme Etkinlik Ölçeği (TDEÖ) ve fonksiyonel mobilite için Süreli Kalk ve Yürü Testi (SKYT) kullanılmştır.
Değerlendirmeler sonucunda HT tanısı almış bireylerde normotansif bireylere göre reaksiyon zamanının istatistiksel olarak daha uzun olduğu belirlendi (tüm p’ler < 0,05). HT tanısı almış bireylerde normotansiflere göre statik denge (gözler açık ve gözler kapalı) (tüm p’ler < =0,001) ve dinamik denge (tüm p’ler < 0,05) sonuçlarının istatistiksel olarak daha düşük olduğu bulundu. HT tanısı almış bireylerin normotansif bireylere göre Süreli Kalk ve Yürü Testi tekli ve ikili görev (bilişsel ve motor) sürelerinin daha uzun, düşme öykülerinin ise daha fazla olduğu saptandı (tüm p’ler < 0,05). Ayrıca hipertansiyon tanısı olan bireylerde denge ve düşme öyküsü arasındaki ilişki incelendiğinde pozitif yönde önemsiz düzeyde ilişki belirlendi.
Çalışma sonucunda, HT tanısı almış bireylerin normotansif bireylere göre reaksiyon zamanın daha uzun, statik ve dinamik dengenin daha fazla azalmış, fonksiyonel mobilitenin daha fazla etkilenmiş ve düşme öyküsünün daha fazla olduğu tespit edildi. Bu sonuçlar doğrultusunda hipertansiyon tanısı alan bireylerin gerekli durumlarda koruyucu rehabilitasyon programlarına, ev-temelli veya fizyoterapist gözetiminde egzersiz programlarına dahil edilmelerinin önemli olduğu kanısındayız.
