Gestasyonel Diyabet Durumunda Probiyotik Takviyesinin Gebelik Komplikasyonları, Beslenme Durumu, Yenidoğan Sağlığı ve Anne Sütüne Etkileri: Tek Merkezli Randomize Kontrollü Çalışma

Abstract
Gestasyonel diyabet vücutta çeşitli patofizyolojik yolaklardan etkilenerek gebelik döneminde en sık görülen sağlık sorunlarından biridir. Gestasyonel diyabet olan kadınlardan doğan bebeklerde de kısa ve uzun süreli komplikasyonlar halk sağlığı için bir tehdit oluşturmaktadır. Literatürde probiyotik takviyesinin gestasyonel diyabeti önlediğine dair veriler bulunmakla birlikte tedavisine yönelik uygulanalar sınırlıdır. Bu çalışma; probiyotik takviyesinin gestasyonel diyabette gelişen komplikasyonlar, antropometrik ölçümler, inflamatuvar belirteç olarak C-reaktif protein (CRP) ve glisemik kontrol üzerine etkilerinin değerlendirilmesi amacıyla yapılmıştır. Tek merkezli yapılan bu randomize kontrollü yapılan çalışmada gestasyonel diyabet tanısı almış 24. gebelik haftasındaki gebeler iki gruba ayrılmıştır. Bir gruba (kontrol grubu) Akdeniz diyetine uyumlu beslenme tedavisi, diğer gruba (probiyotik grubu) ise buna ek olarak (Lactobacillus acidophilus (4,3 × 108 cfu/poşet), Lactobacillus rhamnosus (4,3 × 108 cfu/poşet), Bifidobacterium bifidum (4,3 × 108 cfu/poşet), Bifidobacterium longum (4,3 × 108 cfu/poşet) ve Enterococcus faecium (8,2 × 108 cfu/poşet) probiyotik takviyesi verilmiştir. Gestasyonel diyabette inflamatuvar diyet bileşenlerinin gebelik komplikasyonuna etkisini incelemek amacı ile gebelerin 24. gebelik haftasında diyet inflamatuvar indeksi (Dİİ) skoru belirlenmiştir. Belirlenen Dİİ skorları en düşükten en yükseğe doğru sıralanarak dört quartile ayrılmıştır. Birinci quartil (Q1) anti-inflamatuvar diyeti temsil etmiş, guartiller yükseldikçe diyetin inflamatuvar yükünün arttığı, 4.quartilin (Q4) pro-inflamatuvar diyeti temsil ettiği kabul edilmiştir. Katılımcılara uygulanan diyet ve probiyotik takviyesinin inflamatuvar ve glisemik belirteçlere etkisini saptamak amacıyla 12 hafta süren müdahale sonunda annenin kan CRP, glukoz seviyesi incelenmiştir. Doğumdan sonra yenidoğana uygulanan probiyotik takviyesinin bilirubin düzeyine etki edip sarılık tedavisinde etkili olduğun belirten klinik çalışmalar mevcuttur. Ancak gestasyonel diyabette gebelik döneminde probiyotik takviyesinin bilirubine etkisine yönelik veri bulunmamaktadır. Bu amaçla bebeğin kan bilirubin seviyesi analiz edilmiştir. Ayrıca gestasyonel diyabette gebelik dönemi ağırlık artışının ve yaşına göre iri doğan bebek insidansının sağlıklı gebelikten daha büyük bir endişe yarattığı da bilinmektedir. Müdahalenin antropometrik ölçümlere etkisi de bu amaçla incelenmiştir. 24. haftada besin tüketim kaydı ile hesaplanan Dİİ skoruna diyet bileşenlerinin etkisi incelendiğinde anti-inflamatuvar (Q1) diyet tüketen kadınların diyetle ortalama enerji, karbonhidrat ve yağ alımları; pro-inflamatuvar (Q4) diyet tüketenlerden daha düşük bulunmuştur (p<0.05). Gebelik boyunca vücut ağırlığı artışı kontrol grubunda (ortalama 11.9±2.1) probiyotik gruptan (ortalama 11.4±1.5) yüksek bulunmakla birlikte fark istiksel olarak anlamlı bulunmamıştır (p>0.05). Regresyon analizi ile gebelik öncesi vücut ağırlığı ile 36. gebelik haftasında vücut ağırlığı arasında pozitif ve anlamlı bir ilişki olduğu saptanmıştır (p<0.05). Probiyotik takviyesi alan gruptan doğan bebeklerin kan bilirubin seviyesi daha düşük saptanmıştır (probiyotik grup:6.9± 4.2, kontrol grubu 10.3 ±5.0; p<0.05). Probiyotik takviyesi alan (100.9 ±12.2) ve kontrol grubunun (99.0 ±11.4) maternal kan glikoz seviyeleri arasında fark bulunmamıştır (p>0.05). Çalışmada kontrol grubunda kan CRP seviyesi (1.4± 0.4) probiyotik grubundan (1.35± 0.3) daha yüksek bulunmakla birlikte fark istatistiksel olarak anlamlı bulunmamıştır (p>0.05). Sonuç olarak gestasyonel diyabette oluşacak komplikasyon yükünü azaltmak için Akdeniz diyeti ile uyumlu beslenme tedavisine ilave probiyotik takviyesi uyumlu bulunmuştur. Probiyotik takviyelerinin suş ve konak farklılığından etkilenmesinden dolayı daha geniş bir örneklemde ve iyi planlanmış benzer çalışmalar yapılmalıdır. Gestational diabetes is one of the most common health problems during pregnancy, affected by various pathophysiological pathways. Short-term and long-term complications in babies born to individuals with gestational diabetes also pose a threat to public health. There are data in the literature on the prevention of gestational diabetes with probiotic supplementation, but limited applications for treatment. This study was conducted to evaluate the effects of probiotic supplementation on complications of gestational diabetes, anthropometric measurements, inflammatory markers (CRP) and glycemic control. In this single-center randomized controlled study, individuals diagnosed at 24 weeks of gestation were divided into two groups. One group (control group) was offered only a Mediterranean diet-compatible nutritional treatment, while the other group (probiotic group) was offered a Mediterranean diet-compatible nutritional treatment plus probiotic supplements (Lactobacillus acidophilus (4.3 × 108 cfu/bag), Lactobacillus rhamnosus (4.3 × 108 cfu/bag), Bifidobacterium bifidum (4.3 × 108 cfu/bag), Bifidobacterium longum (4.3 × 108 cfu/bag) and Enterococcus faecium (8.2 × 108 cfu/bag) (NBL Probiotic Gold®, Nobel İlaç, Turkey). Dietary inflammatory index score was calculated in the 24th gestational week of the pregnant women participating in the study. The obtained DII scores were ranked from the lowest score to the highest score and divided into quartiles. The first quartile (Q1) represents the anti-inflammatory diet. As the quartiles increase, the inflammatory load is increasing and 4th quartile (Q4) represents the pro-inflammatory diet. In addition, at the end of the 12-week intervention, some biochemical parameters and anthropometric measurement differences of the mother (CRP, blood glucose level) and the baby (bilirubin) were analyzed. The average energy, carbohydrate and fat intakes of women consuming anti-inflammatory (Q1) diet were found to be lower than those consuming pro-inflammatory (Q4) diet (p<0.05). When the gestational weight gain during pregnancy was examined, a higher weight gain was seen in the control group (11.9±2.1) compared to the probiotic group (11.4±1.5), but this was not found to be statistically significant (p>0.05). A positive and significant relationship was found between pre-pregnancy weight and body weight at 36 weeks of gestation with regression analysis (p<0.05). Bilirubin levels of babies born from the probiotic supplement group were found to be statistically significantly lower (probiotic group: 6.9± 4.2, control group 10.3 ± 5.0; p<0.05). No statistically significant difference was found between maternal glucose values of the probiotic (100.9 ± 12.2) and control groups (99.0 ± 11.4) (p>0.05). In our study, CRP levels were found to be higher in the control group (1.4± 0.4) than in the probiotic group (1.35± 0.3) and no statistically significant difference was found (p>0.05). As a result, medical nutrition therapy for the Mediterranean diet was found to be effective in reducing the burden of complications in GDM. It is recommended that similar studies with large samples and well-planned future studies be conducted due to the fact that probiotic supplements are affected by strain and host differences.
