Gazimağusa Devlet Hastanesi’nde Ameliyat Olacak Hastaların Yakınlarının Bilgi Gereksinimleri ve Bilgilendirilme Durumları

Abstract

ÖZ: Bu araştırmanın amacı; Gazimağusa Devlet Hastanesi’nde ameliyat olacak hastaların yakınlarının, bilgi gereksinimlerinin ve bilgilendirilme durumlarının belirlenmesidir. Araştırma tanımlayıcı – kesitsel araştırma tasarımına uygun olarak yapılmıştır. Araştırma evrenini, Gazimağusa Devlet Hastanesi ameliyathane ünitesinde ameliyat olacak hastaların yakınları ve örneklemini ise, etik kurul izni alındıktan sonraki 4 aylık sürede (15.02.2017 – 15.06.2017) ameliyat olan, araştırma yönergelerini anlayabilme ve tepki verebilme yetisi olan ve bilgilendirilmiş onam formunu imzalayan 106 hastanın yakınları oluşturmuştur. Veriler; ‘‘Hastaların Yakınlarının Tanıtıcı Özellikleri ve Hastaların Yakınlarının Bilgi Gereksinimleri ve Bilgilendirilme Durumlarını İçeren Anket Formu’’ kullanılarak araştırmacı tarafından hastane ortamında, hastası ameliyatta olan hastaların yakınları ile yüz yüze görüşme yapılarak toplanmıştır. Araştırmada verilerin istatistiksel değerlendirilmesinde yüzdelik frekans testi kullanılmıştır. Araştırmadan elde edilen verilere göre; hastaların yakınlarının %46.2'si 50- 65 yaş grubunda, %75.8'i erkek, %83.9'u evli, %39.6'sı okur-yazar değil veya ilkokul mezunu ve %39.6'sı ev hanımıdır. Hastaların yakınlarının ameliyata ilişkin bilgi alma durumları incelendiğinde, %92.5'inin bilgi aldıkları ve bilgi kaynağı olarak %97.9’unun hekimi ifade ettikleri ve tamamının, bilgiyi sözel olarak almak istedikleri saptanmıştır. Hastaların yakınlarının yarıdan fazlasının %54.5’i, ameliyat sonrası; %17.0’sinin ameliyat sırası ve %8.5’inin de ameliyat öncesi döneme ilişkin bilgi alma gereksiniminin olduğu belirlenmiştir. Hastaların yakınlarına, ameliyat öncesi, sırası ve sonrası dönemde bilgi verilmesi gereken konular verilip, bilgilendirilme durumları ve bilgiyi kimden aldıkları sorulduğunda ise, gereksinim duydukları bilgilerin ve bilgiyi hekimden almak isteme oranlarının çok yüksek olduğu saptanmıştır. Cerrahi girişim uygulanacak hastalar için bilgi alınacak ve bilgi alınan tek sağlık profesyonelinin hekim olduğu saptanmıştır. Bu sonuçlar doğrultusunda; cerrahi hemşirelerinin temel sorumlulukları olan ve hastanın cerrahi girişim için başvurmasıyla başlayan, ameliyat öncesi hazırlık, ameliyat sonrası bakım, taburculuk planlaması ve bu aşamalara yönelik hasta ve yakınlarının bilgilendirilmesi konusunda, hemşirelerin bilgilerinin geliştirilmesi ve sürekli eğitimlerle, güncellenmesi önerilmiştir. Anahtar Kelimeler: Hasta Yakını, Bilgi Gereksinimi, Bilgilendirilme Durumu, Cerrahi HemşiresiThe purpose of this study is to determine the information needs of the relatives of the patients to be operated on at Gazimağusa State Hospital well as their state of being informed. The study was conducted in compliance with a descriptive – cross-sectional design. The population consisted of the relatives of the patients to be operated on at Gazimağusa State Hospital, while after receiving permission from the board of ethics, the sample consisted of relatives 106 of the patients who had operations in a 4-month period (15.02.2017-15.06.2017) who had the ability to understand and respond to the instructions of the study and signed informed consent forms. The data were collected by the researcher in the hospital environment via face to face interviews with the relatives whose patients were in operation using the “Questionnaire Form for Descriptive Characteristics of Patient Relatives and Their Needs for Information and Their State of Being Informed”. Ethics Board approval was received for conducting the study. Percentages and frequency test were used to analyze the data. According to the data obtained in the study 46.2% of the relatives were in the age range of 50-65, 75.8% were male, 83.9% were married, 39.6% were illiterate or primary school graduates and 39.6% were housewives. When the states of the relatives of being informed were investigated, it was found that 92.5% received information and 97.9% of the participants stated the doctor as their source of information, while all participants stated that they would like to receive information verbally. More than half of the relatives of patients (54.5%) stated they needed information for the post-operative period, while 17.0% said they needed information for the period during the operation and 8.5% said they needed information about the pre-operation period. When the relatives were provided with the subjects they need to be informed about in the periods before, during and after operations and they were asked about their state of being informed and from whom they received information, it was found that they had very high rates of information they needed and wishes to receive information from the doctor. It was found that the only healthcare professional who was stated to provide and have provided information for patients to be operated on was the doctor. In the light of these results; it is recommended to improve the knowledge of nurses and update it through continuous training for planning preparations before operations, post-operative care and discharging, and informing the patient and their relatives regarding these stages.
