Obez Çocuklarda Kor Stabilizasyon ve PNF Egzersizlerinin Kor Kas Aktivasyonu Üzerine Etkileri: Randomize Kontrollü Çalışma

Abstract
Bu çalışmanın amacı obez çocuklarda kor stabilizasyon ve propriyoseptif nöromüsküler fasilitasyon (PNF) egzersizlerinin kor kas aktivasyonu, lomber kor kas enduransı, lomber eklem pozisyon hissi, denge, beden imajı ve yaşam kalitesi üzerine etkilerini incelemekti. Tek kör randomize kontrollü gerçekleşilen çalışmamıza 80 obez çocuk katıldı ve randomize olarak üç gruba ayrıldı. Birinci gruba (Kor) kor stabilizasyon, ikinci gruba PNF egzersizleri 8 hafta boyunca haftada 3 gün uygulandı. Kontrol grubu olan üçüncü gruba herhangi bir tedavi uygulanmadı. Katılımcıların Kor kas aktivasyon düzeyi (Sahrmann Kor Stabilite testi), lomber kor kas enduransı (McGill kor endurans testleri), lomber eklem pozisyon hissi (lazer imleci), statik dengesi (Tek Ayak Üzerinde Denge Testi), dinamik dengesi (Y denge testi), beden imajı (Çocuk Beden İmajı Ölçeği) ve yaşam kalitesi (Pediatrik Yaşam Kalitesi Ölçeği) tedavi öncesi, tedavi sonrası ve 3 aylık izlem süresinin sonunda değerlendirildi. Tedavi sonrası ve izlemde kor aktivasyon düzeyi, kor kas endurans değerleri, lomber eklem pozisyon hissi, gözler açık ve kapalıyken tek ayak üzerinde denge süreleri ve yaşam kalitesi gruplar arasında istatistiksel olarak anlamlı düzeyde farklıydı (p<0,05). Y denge testi sonuçları ve beden imajı ölçeğinden alınan puanlar açısından gruplar arasında istatistiksel olarak anlamlı bir fark bulunmadı (p>0,05). Tüm sonuç ölçümlerinde Kor grubunda klinik etki büyüklükleri daha yüksekti. Obez çocuklarda kor stabilizasyon ve PNF egzersizleri kor kas aktivasyon düzeyi, lomber kor kas enduransı, lomber eklem pozisyon hissi, denge ve yaşam kalitesini geliştirmektedir. Klinik açıdan kor stabilizasyon egzersizleri obez çocuklarda daha etkilidir. Etkiler orta vadede azalmaktadır. The aim of this study was to investigate the effects of core stabilization and proprioceptive neuromuscular facilitation (PNF) exercises on core muscle activation, lumbar core muscle endurance, lumbar joint position sense, balance, body image and quality of life in obese children. In our single-blind randomised controlled study, 80 obese children were randomly divided into three groups. The first group received core stabilization and the second group received PNF exercises 3 days a week for 8 weeks. The third group, which was the control group, received no treatment. Participants were evaluated with core activation (Sahrmann Core Stability test), lumbar core muscle endurance (McGill core endurance tests), lumbar joint position sense (laser cursor), static balance (Single-Leg Standing Balance test), dynamic balance (Y balance test), body image (Children's Body Image Scale) and quality of life (Pediatric Quality of Life Scale) before and after treatment and at the end of the 3-month follow-up period. After treatment and during follow-up, core activation, core endurance, joint position sense, one-leg stand times with eyes open and closed and quality of life were statistically significantly different between the groups (p<0.05). There was no statistical difference between the groups in terms of Y balance test results and body image scores (p>0.05). Clinical effect sizes were higher in the Core group in all outcome measures. Core stabilization and PNF exercises can improve the level of core muscle activation, lumbar core muscle endurance, lumbar joint position sense, balance and quality of life in obese children. Clinically, core stabilization exercises are more effective. The effects decline in the medium term.
