Hemşirelik Öğrencilerine Uygulanan Akupresur Terapi Yönteminin Sınav Kaygısı ve Yaşamsal Bulgular Üzerine Etkisi: Randomize Kontrollü Deneysel Bir Çalışma

Abstract
Bu araştırma; hemşirelik öğrencilerine uygulanan akupresur terapi yönteminin sınav kaygısı, sistolik ve diyastolik kan basıncı, kalp atım hızı ve arteriyel oksijen satürasyon değeri üzerine etkisini incelemek amacıyla randomize kontrollü deneysel tasarım tipinden planlanmıştır. Araştırmanın evrenini; 2023-2024 akademik yılı güz döneminde Doğu Akdeniz Üniversitesi Sağlık Bilimleri Fakültesi Hemşirelik Esasları dersine kayıtlı 2. sınıf öğrencileri oluşturmuştur. Araştırmanın örneklemini ise, araştırmaya dahil edilme kriterlerine uyan ve araştırmaya katılmayı kabul eden deney (n=59) ve kontrol (n=59) olmak üzere 118 öğrenci oluşturmuştur. Araştırmada veriler, öğrencilere yönelik bilgileri içeren “Kişisel Bilgi Formu” , “Durumluk Sınav Kaygısı Ölçeği ” ve “Yaşamsal Bulgular Takip Çizelgesi” kullanılarak toplanmıştır. Verilerin istatiksel analizinde yüzdelik, Pearson ki kare testi ve Fisher kesin ki-kare testi kullanılmıştır. Öğrencilerin 85,6’sının kaygının sınavları olumsuz etkilediğini belirttiği, 24,6’sının yazılı, %58,5’inin uygulama sınavına kendini hazır hissetmediği ve %85,6’sının uygulama sınavının kötü geçmesinden korktuğu belirlenmiştir. Sınav kaygısı ile baş etmede öğrencilerin %27,1’inin nefes egzersizi, %22,9’unun meditasyon yaptığı, %4,2’sinin müzik dinlediği, %8,5’inin diğer bazı yöntemleri uyguladığı saptanırken; %37,3’ünün sınav kaygısını yönetmede hiçbir yöntem uygulamadığı saptanmıştır. Çalışmanın ön test sonuçları incelendiğinde, öğrencilerin orta düzeyde durumluk sınav kaygısına sahip oldukları belirlenmiştir. Hemşirelik öğrencilerinin son test Durumluk Sınav Kaygısı Ölçeği ölçümlerinde ise kontrol grubu öğrencilerinin kaygı düzeylerinde artış görülürken, deney grubu öğrencilerinin genel durumluk sınav kaygı düzeylerinin azaldığı görülmüştür. Deney grubundaki öğrencilerin son test ölçümlerinde durumluk sınav kaygılarında istatistiksel olarak anlamlı derecede düşüş olduğu belirlenmiştir. Yapılan çalışmada deney grubunda sistolik ve diyastolik kan basıncı düşüşünün kontrol grubundan anlamlı düzeyde fazla olduğu saptanmıştır. Benzer şekilde deney grubunda kalp atım hızı düzeyindeki düşüşün kontrol grubundan anlamlı düzeyde fazla olduğu saptanmıştır. Sonuç olarak akupresur terapi yöntemi hemşirelik öğrencilerinin sınav kaygısı düzeyini azalttığı görülmüştür. Ayrıca sistolik ve diyastolik kan basıncı ve kalp atım hızını da düşürmede etkisi görülmüştür. Bu noktada öğrencilerin sınav kaygısını azaltmaya yönelik akupresur terapi uygulamanın kullanılması önerilir. The objective of this study was to examine the effect of acupressure therapy on nursing students' anxiety levels, blood pressure, heart rate, and arterial oxygen saturation. To this end, a randomized controlled experimental design was employed. The study population consisted of second-year students enrolled in the Principles of Nursing course at Eastern Mediterranean University Faculty of Health Sciences during the fall semester of the 2023-2024 academic year. The study sample consisted of 118 students, including 59 experimental and 59 control subjects who met the inclusion criteria and consented to participate in the study. The data were collected using the descriptive information form, the State Test Anxiety Scale (STAS), and the Vital Signs Follow-up Chart. The statistical analysis of the data was conducted using the percentage, Pearson chi-square test, and Fisher's exact chi-square test. The study found that 85.6% of the students indicated that anxiety had a negative impact on their performance in the examinations. Additionally, 24.6% of the students reported feeling unprepared for the written examination, while 58.5% indicated similar feelings regarding the practical examination. Furthermore, 85.6% of the students expressed apprehension about the potential outcome of the practical examination. The results indicated that 27.1% of the students engaged in breathing exercises, 22.9% utilized meditation, 4.2% listened to music, and 8.5% employed other strategies to cope with test anxiety. Conversely, 37.3% of the students did not utilize any method to manage test anxiety. Analysis of the pretest results indicated that the students exhibited a moderate level of anxiety. Although an increase was observed in the STAS scores of the control group, there was a statistically significant decrease in the post-test STAS (State Test Anxiety Scale) scores of the intervention group. The study revealed that the experimental group exhibited a significantly greater decrease in systolic and diastolic blood pressure levels compared to the control group. Similarly, the experimental group demonstrated a significantly greater decrease in heart rate compared to the control group. The results demonstrated that acupressure therapy effectively reduced the level of exam anxiety experienced by nursing students. Additionally, this approach was observed to have a beneficial impact on systolic and diastolic blood pressure and heart rate. Based on these findings, it is recommended that acupressure therapy be considered as a potential intervention to mitigate exam anxiety among students.
